The goal was to change that, capturing these real-time, ground-level insights and feeding them directly into a structured clinical intelligence layer: the Continuum Health Intelligence Platform (CHIP) and Predictive Wellness Stratification Model (PWSM).   • Synchronization with centralized Holistic Wellness Data (HWD) scoring and the Risk Stratification Engine (RSE)
  • A “Notify” feature to rapidly escalate emerging concerns to the right stakeholders
  • Role-based dashboards for Observers, Caregivers, Geriatric Care Specialists, and Primary Care Providers
  • Enterprise-grade security using TLS 1.2+, AES-256 encryption, and AWS infrastructure
  • Accessibility compliance aligned with WCAG 2.1 AA standards
